MyMedList, a division of RJR Enterprises of SW FL, LLC, provides a user-friendly, electronic means to create, update, and communicate a patient’s list of medications across the continuum of care with the goals of improving quality of life, providing a layer of security against unintended harm, reducing hospital admissions and readmissions, and bringing about a level of comfort to clients and their caregivers. Our company will utilize a team of highly motivated healthcare professionals who share a passion for patient and medication safety.

Founder and CEO, Jason Rice, is a licensed pharmacist in Florida and North Carolina, and possesses over 25 years of experience as a pharmacist and over 30 years as a healthcare professional. Over the past year, he has served as a Clinical Pharmacist with an emphasis on Hospital Medication Reconciliation. Additionally, he served 13 years as System Director of Pharmacy for a major healthcare system in the Northwest portion of North Carolina, where he designed and implemented the system’s Medication Reconciliation Program. This experience allowed for the identification of major gaps in the transition of care for patients, which unfortunately have led to tremendous numbers of medication errors and unintended patient harm.

MyMedList aims to change this decades-long, broken system by providing a source of tools and professional expertise that relieves patients and their caregivers the burden of keeping up with lengthy, ever-changing medication lists, as well as alleviate concerns that the most up-to-date information is available to all their providers in a timely manner.
