Jason Rice, is a licensed pharmacist in Florida and North Carolina, and possesses over 26 years of experience as a pharmacist and over 30 years as a healthcare professional. Over the past three years, he has served as a Director of Pharmacy, and prior as a Clinical Pharmacist with an emphasis on Hospital Medication Reconciliation. Additionally, he served 13 years as System Director of Pharmacy for a major healthcare system in the Northwest portion of North Carolina, where he designed and implemented the system’s Medication Reconciliation Program. This experience allowed for the identification of major gaps in the transition of care for patients, which unfortunately have led to tremendous numbers of medication errors and unintended patient harm.
